Before Appointment

New Patient Intake

These forms allow you to provide important medical history, insurance details, and contact information before your appointment.

You can send them in ahead of time, print and bring them with you, or, if not, arrive 15 minutes early to fill them out in person.

Reminder

You’ll receive timely notifications via text, email, or phone to help you prepare for your visit and avoid any scheduling conflicts.

Please note, we kindly ask for at least 24 hours’ notice if you need to cancel or reschedule your appointment.

Day of:

Parking

Street parking at our office can be limited, so please allow extra time to find a spot. For your convenience, the building offers a parking garage, but rates may vary and are not managed by our office. We recommend arriving early to ensure a smooth start to your appointment.

Form of Payment

We accept a variety of payment options for your convenience, including credit and debit cards, cash, checks, and HSA (Health Savings Account) cards.

Your Visit with Dr. Chou:

Chart review with
Medical Assistant

Before seeing Dr. Chou, you’ll meet with one of our skilled medical assistants for a chart review. During this time, they’ll update your medical history, review your current medications, and address any initial concerns to ensure your visit is thorough and efficient.

Care Plan with
Dr. Chou

After your evaluation, Dr. Chou will work with you to create a personalized care plan tailored to your specific needs. This plan will address your concerns, outline treatment options, and set clear steps to help you achieve optimal foot and ankle health.

Follow Up
(usually 1 to 3 as needed)

To ensure the best outcomes, we may schedule 1 to 3 follow-up appointments as needed. These visits allow us to monitor your progress, make any necessary adjustments to your care plan, and ensure your recovery is on track.

  • Take US-101 South
  • Continue on Main Post Tunnel/Presidio Pkwy
  • Continue onto Richardson Ave
  • Continue onto Lombard Street (briefly)
  • Turn right onto Broderick Street
  • Turn left onto Vallejo Street
  • Turn right onto Webster Street
  • Office will be on your left

Driving From the South (South San Francisco)

  • Take US-101 North
  • Continue on US-101 N/Central Fwy
  • Turn right onto Market Street
  • Use the left 2 lanes and turn slightly left toward
    Franklin Street
  • Continue onto Franklin Street
  • Turn left onto Pine Street
  • Turn right onto Webster Street
  • Office will be on the right

Preparing For Your
First Visit

Whenever we welcome new patients, we are required to take and enter their information into our records. A great way to save time waiting for your first appointment is by filling out your registration form in advance. You can view and print out the form by clicking the button below.

signed or returned to our office.

What to Bring

In addition to your registration form, please also bring the following items with you to your first appointment, as applicable:

  • Your driver’s license (or other photo ID).
  • Any insurance card(s) you may have.
  • Copies of X-rays, MRIs, or CT scans related to your current condition.
  • Copies of other medical records related to your current condition.
  • The shoes that you wear most often, or shoes that cause you problems.
  • Any over-the-counter or custom orthotics you may be using.
  • A list of all your questions or concerns. It can be easy to forget what you would like to ask when you arrive at the office, and we want to ensure that all your questions are answered!
